The Agency for Healthcare Research and Quality (AHRQ) and its contractor, Westat are designing Version 2.0 of the AHRQ Hospital Survey on Patient Safety Culture and a Health Information Technology (HIT) Patient Safety supplemental item set.

The AHRQ is looking for hospitals with at least 50 licensed beds and have implemented an electronic health record (EHR) system to participate in a pilot test, which involves administering the survey to your providers and staff. Participating hospitals will receive an individual feedback report comparing their results to other pilot sites and how their scores may differ between the original Hospital safety culture survey and this updated 2.0 version. In addition, Westat can automatically upload the original survey (HSOPS 1.0) data collected in the pilot to the Hospital database when it opens on June 1, 2017 provided participating hospitals sign a Data Use Agreement.

If you would like to learn more, or are interested in participating, please send an email to with “Pilot Test” in the subject line.
