Closing the Loop on Patient Referrals in Health Care

Faulty referral systems can harm patients when they lead to missed or delayed diagnosis and treatment, and in her latest post on, Tejal Gandhi, MD, MPH, CPPS, IHI Chief Clinical and Safety Officer, shares her own experience with the referral process. During her time as a primary care physician, there was no industry standard for keeping track of referrals, yet it is vital to have protocols in place.


The recent IHI report, Closing the Loop: A Guide to Safer Ambulatory Referrals in the EHR Era, outlines a nine-step, closed-loop process in which all relevant patient information is communicated to the correct person through the appropriate channels and in a timely manner.
