This presentation will focus on Abington Jefferson Health System’s experience in implementing the recommendations of NPSF’s recent report, RCA2: Improving Root Cause Analyses and Actions to Prevent Harm. Abington Jefferson, a health care system in Pennsylvania that is comprised of 5 hospitals, 13 outpatient and urgent care centers, and numerous physician practices, has been working in collaboration with the Joint Commission to integrate the recommendations of the RCA2 report with the Joint Commission requirements, specifically around sentinel events. 


Abington will cover the following topics during their presentation:

  • Leadership/Governance involvement in understanding and supporting cause analysis
  • Strategies for compliance to Joint Commission requirements, issues of review ‘capacity’ and commitment to preoccupation with ‘risks’ to safety in the absence of known harm.
  • RCA timing and team membership
