This presentation will focus on Aurora Health Care’s experience in implementing the recommendations of NPSF’s recent report, RCA2: Improving Root Cause Analyses and Actions to Prevent Harm. Aurora Health Care, a health care system in the Wisconsin-area that serves over 1.2 million patients annually, is in the process of developing a process and systems to educate each of the quality and safety teams in its 15 hospitals on the updated root cause analysis process. They have developed tools, including a prioritization matrix and RCA2 checklist, to simplify the process and improve consistency. Aurora Health Care will be joined by Terry Fairbanks, a nationally-recognized patient safety expert and member of the workgroup that developed the RCA2 report.
