Gap Analysis: Patient & Family Partnerships

A gap analysis should be conducted to assess the gaps in the current state versus future state. It is recommended that the flow charting be completed along with the gap analysis for most favorable future state. A confluence of process, provider practice and product application are imperative for creating optimal standards of safe care.

EXAMPLE: Delay in Obtaining and Sharing Lab Results
Current State
  • Once lab is drawn and read, results are printed in paper form on the nursing unit.
  • No one is designated to file the results in the patient chart.
  • No one calls / communicates the results to the physician.
Future/Ideal State
  • Results should be entered electronically into the patient chart.
  • Results should be shared / communicated in a timely manner to the physician.
