Gap Analysis: Patient & Family Communications

A gap analysis should be conducted to assess the gaps in the current state versus future state. It is recommended that the flow charting be completed along with the gap analysis for most favorable future state. A confluence of process, provider practice and product application are imperative for creating optimal standards of safe care.

  • Fire alarms went off in the early morning, resulting in loud noise.
  • Staff walked around closing doors and performing safety checks.
  • Received several patient complaints and concerns; they felt worried and uninformed, One patient fell trying to get out of their room.
Future/Ideal State Keeping patients informed by communicating current state to all patients.
