Universal Patient Compact Survey

NPSF has developed a two-part survey to assist organizations who are seeking to gauge how closely their practices adhere to the principles of the Universal Patient Compact.  Part 1 of the survey is  to be completed by representatives of the organization, and asks for the staffs’ perspective. Part 2 is  to be completed by patient representatives and asks the same questions. Comparing answers from both respondents will likely reveal gaps that indicate areas of disconnect and opportunities for dialogue and improvement.

NPSF greatly appreciates the participation of health care staff and patients who complete the respective two parts of the survey online.  As we expect the role of patients and families to continue to evolve, NPSF will review and reaffirm these principles annually during Patient Safety Awareness Week.

You organization should only complete each part of the survey (Part 1 and Part 2) once per year.
