By following best practice protocols enabled through the use of health information and technology, ambulatory practices can reduce negative patient outcomes, more effectively diagnosis chronic disease, and prevent harm. This webinar will detail a series of best practices and technical applications which help mitigate potential risks to patient safety and improve quality within a typical ambulatory care setting.

Learning Objectives:

  • Adopt a nine-step, closed-loop referral process in which all relevant patient information is communicated to the correct person through the appropriate channels and in a timely manner.
  • Develop a remote hypertension monitoring program for diagnosis and treatment of high blood pressure.
  • Identify key process-based quality measures and develop a system for using quality data to drive care improvement.
